The Sound Your Knuckles Make
Donald Unger cracked the knuckles of his left hand — and only his left hand — twice a day for more than sixty years. The right hand was his control group. When he published the results in a letter to Arthritis & Rheumatism in 1998, he'd earned an Ig Nobel Prize and, more to the point, produced a one-man longitudinal study: no arthritis in either hand.
The knuckles that got cracked for six decades looked, on X-ray, exactly like the ones that didn't.
That single stubborn man answered the question everyone asks about knuckle cracking, and the answer is: it doesn't cause arthritis. The myth survives anyway, passed down like a family recipe, mostly because the sound is so disproportionate to the size of the joint. A crack that loud feels like it must be doing damage.
But knuckle cracking is a narrow slice of a much bigger subject. Joints make all kinds of noises — pops, cracks, snaps, clicks, grinds, and a low gravelly creak the doctors call crepitus. Some are gas. Some are tendons. Some are the early sound of cartilage wearing thin. The trick is telling them apart, because the two most common noises sit at opposite ends of the should-I-worry spectrum.
A pop, by itself, in a joint that doesn't hurt: almost certainly fine. A grind, especially one with a feeling of sand under the kneecap: that one's worth paying attention to. The rest of this article is about how to tell which is which — and what, if anything, to do about it.

Where the Pop Actually Comes From

The pop is gas. More precisely, it's cavitation — the sudden collapse of a gas bubble inside the fluid that fills every movable joint. Synovial fluid is thick and slippery, like raw egg white, and it carries dissolved gases: mostly carbon dioxide, some nitrogen, a little oxygen.
When you pull a joint apart — bending a finger back, or stretching a knee into full extension — you drop the pressure inside that sealed fluid chamber. The dissolved gas comes out of solution and forms a bubble. Then, in less than a heartbeat, that bubble collapses, and the collapsing cavity sends a shockwave through the fluid. That's the crack.
For decades this was argued about, because nobody could see it happen. An X-ray of a cracking joint was too slow, and the bubble, if it existed, formed and vanished in milliseconds. Then in 2015 a group at the University of Alberta put a hand inside a real-time MRI scanner and pulled the fingers until they cracked.
The footage is uncanny: a bright cavity blooming open inside the knuckle at the exact instant of the pop. The crack, it turns out, isn't the bubble forming. It's the bubble collapsing.
That detail matters for a practical reason. After a joint pops, the gas has to re-dissolve back into the fluid, which takes roughly twenty minutes. That's why you can't crack the same knuckle twice in a row, and why a joint that feels 'stuck' — as if it needs to pop — is usually just a joint that hasn't moved through its full range in a while. The pop isn't realigning anything. It's a pressure event, nothing more.
- The 'twenty-minute rule' is a good self-check: if a joint feels like it needs to pop again within minutes, what it actually needs is movement, not another crack.
- Don't force a crack that doesn't come easily. Pulling hard to manufacture a pop is how people strain the soft tissue around a joint.
Crepitus Is Not a Crack
Crepitus sounds like the crack, but it isn't. Crack a knuckle and it's a single, clean snap. Crepitus is a crunch — the noise of two rough surfaces dragging over each other, or of a tendon rolling across a bony bump. People describe it as gravel, as sand, as rubbing two pieces of stale bread together.
There are two common kinds. The first is soft-tissue crepitus: a snapping or clicking when a tendon or ligament flicks over a bony prominence. Think of the snapping hip a runner feels, or the click in a shoulder when you raise your arm overhead. This kind is often loud, often painless, and usually a quirk of anatomy — the tendon just happens to sit a little tighter than the pulley it rides over.
If it doesn't hurt, it's generally not a problem.
The second kind is the one worth taking seriously: joint crepitus, a fine grinding felt inside the knee or under the kneecap, especially with the hand placed on the joint. This is the sound of cartilage that's no longer perfectly smooth. Cartilage is the white, glassy cap on the ends of bones, and when it's healthy it's slicker than ice — a joint lined with good cartilage moves almost silently.
When that surface starts to roughen or thin, the two sides no longer glide. They grind. People often feel it as much as they hear it, a grittiness that travels up through the hand.
The honest caveat: crepitus by itself, with no pain, is extremely common and not automatically a disease. Studies of knees find crepitus in a large share of people with no pain and no arthritis on imaging. But crepitus plus pain, or crepitus that's new and getting louder, is the combination that tends to show up alongside actual cartilage change. The sound is a clue.
It's the pain that tells you whether the clue matters.
- A snap that happens once per movement, at a predictable angle, is usually a tendon. A fine grind that happens through the whole arc of movement is more likely cartilage.
- Track the noise for a week before you worry: note when it happens, whether it hurts, and whether it's getting louder. A dated log beats a vague memory when you talk to a clinician.
When the Noise Is Talking to You

Here's the short version, the one I'd want taped to the fridge. Noise alone — a crack, a pop, a click — in a joint that feels fine: almost always benign. You don't need to do anything except maybe stop wincing. Noise plus any of the following is a different story, and it's worth getting looked at.
Pain. If the sound comes with pain — sharp, dull, or an ache that lingers after — that's the joint telling you the surfaces or the soft tissue around it are irritated. Swelling or warmth. A joint that looks puffy, feels hot, or holds fluid after activity is inflamed, and inflammation plus noise is not a self-treatment situation. Locking or catching.
If a joint sticks, locks, or feels like it gives way when you move it, something mechanical is happening — a loose piece of cartilage, a torn meniscus, a ligament that isn't doing its job. That's an orthopedist's domain, not a stretching routine's.
And stiffness that outlasts the morning: a little stiffness on waking is normal, but stiffness that persists past half an hour, or that comes with night pain, is a classic flag for inflammatory arthritis, which needs medical care — not a heating pad and optimism.
There's also the matter of change over time. A noise you've had since your twenties, unchanged, painless — that's your normal. A noise that appeared last month and is getting louder, or that now comes with a sensation you didn't have before — that's new information. The body doesn't keep score forever, but it does keep a running log. Pay attention when the log adds a line.
- The red-flag list, memorized: pain, swelling, warmth, locking, giving way, stiffness past 30 minutes, night pain. Any one of these plus a noise means get it seen.
- Take a short video of the noise before an appointment if you can capture it. Describing a sound over the phone is harder than playing one.

A Noisy Joint in Chinese Medicine
Classical Chinese medicine doesn't have a category for 'cracked knuckles,' but it has a lot to say about noisy joints. The relevant framework is Bi syndrome — painful obstruction — the umbrella term the Yellow Emperor's Inner Canon uses for anything that blocks the free movement of Qi and Blood through the joints and channels. A joint, in this system, is a place where Qi and Blood are supposed to flow.
When they can't — when Cold, Damp, or Wind lodges there, or when the body simply runs low on the substances that keep joints lubricated — the joint stiffens, aches, and, in the classical descriptions, makes sound.
Two patterns show up most often around joint noise. The first is stagnation: Qi and Blood bound up in the channels, so the joint loses its smooth glide and starts to click and catch. The classical remedy is movement and warmth — disperse the stagnation, and the noise softens. The second is deficiency: the Liver Blood and Kidney essence that nourish the sinews and bones run thin with age or overwork.
When the Liver can't keep the sinews supple and the Kidney can't keep the bones and marrow filled, the joints dry out — and a dry joint, in TCM as in a machine, runs louder than a lubricated one.
What's striking is how well this maps onto the modern picture. The lubrication TCM calls jin ye — the body's fluids — has a direct physical counterpart in synovial fluid and the water held inside healthy cartilage.
The instruction to 'move to disperse stagnation' is, mechanically, how joints actually stay fed: cartilage has no blood supply of its own, so it depends on the compression and release of movement to squeeze nutrients in and waste out. A still joint is a starving joint.
The old texts didn't know about cartilage diffusion, but they reached the same conclusion from the other direction — that a joint left still and cold goes stiff, and a joint kept warm and moving stays quiet.
- TCM's two noisy-joint patterns map cleanly onto two self-care levers: stagnation wants movement and warmth; deficiency wants nourishment (rest, hydration, and building the muscle around the joint).
- Notice whether your joint feels worse in cold or damp weather. Cold-aggravated stiffness is the classical Cold-Bi signature, and it responds to warmth before movement.
The Fluids That Should Be There
This is the part worth understanding, because it's where the old and new maps actually line up. Cartilage is the smooth cap that lets bones glide, and it's avascular — no blood vessels run through it.
In a healthy joint, the cartilage gets its nutrition the way a sponge gets water: when you load the joint, fluid is squeezed out; when you unload it, fluid is drawn back in, carrying the molecules cartilage needs to repair itself. Movement is the pump. Sit all day, and the pump doesn't run.
Synovial fluid, the egg-white stuff, does the lubricating. It's rich in hyaluronic acid, a molecule that makes the fluid viscous and slippery. In a healthy joint it coats the cartilage so the two surfaces slide almost without friction. With disuse, the fluid thins and the cartilage loses its slick. With aging and wear, the fluid can break down faster than the joint can replace it.
And here's the thing most people get backwards: cracking a joint doesn't use up its lubrication. The pop is just gas. What actually thins the fluid is inactivity and inflammation — not the occasional crack.
Warmth does something real here too. Heat dilates the small vessels around the joint, brings blood to the capsule and surrounding muscle, and makes the collagen in the soft tissue more pliable. That's why a stiff joint moves better after a hot shower or a heating pad, and why cold weather makes it worse — the fluid itself gets more viscous when it's cold, like honey left in the fridge.
The TCM instruction to warm the channels and move the Blood isn't a metaphor. It's a pretty direct description of what a stiff, cold, under-lubricated joint needs.
- Think of joint nutrition as a pump, not a deposit. No supplement 'feeds' cartilage the way movement does — cartilage has no blood supply and can only take in nutrients when it's loaded and unloaded.
- A hot shower before morning movement is the cheapest joint therapy available. Ten minutes of warmth changes the viscosity of the fluid before you ask the joint to work.
Fear Makes the Noise Worse Than the Joint Does
There's a well-documented loop here that nobody talks about, and it matters more than most supplements. When a joint cracks or grinds, people get nervous. They move less, or they brace — tensing the muscles around the joint to 'protect' it. The bracing changes how the joint loads. The avoidance means the joint stops getting the movement it needs. The stiffness climbs.
And the noise, paradoxically, often gets louder, because a stiffer joint with tighter surrounding muscle produces more of it. The fear of the noise becomes the thing that feeds the noise.
Physiotherapists have a name for this: fear-avoidance behavior, and it's one of the strongest predictors of poor outcomes in joint and back pain. People who interpret a body sensation as damage and then avoid it tend to do worse than people who interpret the same sensation as a quirk and keep moving.
The joint, in most cases, can't tell the difference between your fear and your attention — it just responds to whether it gets used.
This is where the honest framing earns its keep. The crack is gas. The click is usually a tendon. The grind is cartilage, and even that is common and often painless. None of these are instructions to stop moving. The reset below is really a way of breaking the loop — of giving you a safe, structured way to keep the joint moving so the noise can settle instead of compounding.
One practical trick that helps people more than it should: narrate the noise instead of flinching at it. 'Gas bubble. Not damage.' 'Tendon over bone. Not damage.' 'Grind — I'll note it, but it doesn't hurt.' It sounds silly. It also short-circuits the startle that drives the bracing, and the bracing is the part that actually wears the joint. The sound isn't doing the damage. Your reaction to it might be.
- If you notice yourself bracing or limping to avoid a noise, that's the loop to break. The noise is rarely the problem; the compensation pattern can become one.
- A noisy joint that doesn't hurt is a signal to keep it moving, not a signal to wrap it in cotton. Use it, within comfort, every day.
A Two-Week Reset for Quiet Joints
None of this is a promise that your knees will stop sounding like bubble wrap. What it is, is a sensible reset for the most common cause of bothersome joint noise: joints that have gone still, cold, and stiff. Two weeks, four habits, zero gimmicks.
First, move the joint through its full range, daily. The goal is range, not load. For a noisy knee, that's ten slow, unweighted knee bends and straightens each morning, heel slides if you're lying down, or a ten-minute walk if the joint tolerates it. The point is to run the pump — to squeeze fluid in and out of the cartilage and remind the joint what its full arc feels like.
People avoid this because the noise makes them nervous. The noise is not the damage. Avoidance is.
Second, warm it before you move it. Five to ten minutes of a heating pad, a hot shower, or a warm compress on the joint before activity makes the fluid less viscous and the soft tissue more willing. If the joint is visibly hot, swollen, or acutely injured, skip the heat — that's the one situation where warmth is the wrong call.
Third, build the muscle around the joint. For knees, that means the quadriceps — the muscle group on the front of the thigh that acts as the joint's built-in shock absorber. Stronger quads mean less load dumped directly onto the cartilage with every step. Wall sits, straight-leg raises, and shallow squats are the boring, effective trio.
This is the single most evidence-backed lever for knee comfort, and it costs nothing.
Fourth, hydrate like it's a joint issue, because it partly is. Cartilage is mostly water, and a body running chronically dehydrated is a body with less fluid available to keep the joints slick. You don't need to drown yourself. You need to not be thirsty, consistently.
- Do the range-of-motion work first thing in the morning, after warmth, when the joint is at its stiffest — that's when it needs the pump most.
- Quadriceps work is the highest-return habit on this list. Two sets of wall sits, three times a week, is a real intervention for a noisy knee, not a token.
- If a joint locks, swells, or aches at night, stop the self-treatment and see someone who can image it. Two weeks of gentle movement won't fix a mechanical problem.

The Honest Bottom Line
Joint noise scares people because it sounds like damage, and most of the time it isn't. A crack is gas. A click is usually a tendon. A grind is cartilage — and even that, without pain, is common and not a sentence to anything. The dividing line, over and over, is pain. Noise plus pain, swelling, locking, or warmth is information you should act on.
Noise alone is something you can usually ignore, and maybe even learn to like: it's your joints confirming they still move.
The deeper point is that quiet joints are a side effect of something else — of staying warm, staying hydrated, and, above all, staying in motion. Cartilage has no blood supply. It depends on movement the way lungs depend on air. A joint you keep still, keep cold, and keep weak will get louder. A joint you move, warm, and strengthen will often settle down.
Whether it falls silent entirely — that's partly luck, partly anatomy, partly the years. But the direction of travel is up to you. And if the noise ever stops being a quirk and starts being a message — you'll know, because it'll bring pain along with it.
